We are currently recruiting for a Care Coordinator to support our commercial home care operation in the Perth Midland area. Our services include, aged care, transitional care, hospital in the home, disability and palliative care. Right at Home is a leading home care provider with over 512 offices around the world. 

The successful candidate will have a strong background in Community Aged Care. They will thrive working in a close-knit team environment and have a passion for ensuring their team delivers the highest quality in client care.

This position would be responsible for providing quality care services, recruitment and training of carers. The role would involve monitoring operational performance and ensuring that service levels are being met.

The role would involve coordinating and allocating your care team and ensuring that client needs are met. You will also be responsible for assessment and management of HCP and private clients.

Duties include: 

  • Care Assessments
  • Holistic outcomes driven Care Planning
  • Management of complex cases
  • Carer recruitment and training
  • Operation of our Enterprise management and mobile care management system
  • Quality assurance

Qualifications

  • Driver’s License and comprehensively insured dependable private vehicle.
  • Current National Police Clearance.
  • AHPRA-Nurse Registration a requirement.
