Care Coordinator – Aged, Disability & Transitional Care

This is a rewarding and varied role where you will focus on case managing our home care packages and other services for our wide range of community-based clients.

A typical day will consist of identifying  needs and requirements, conducting in-home assessments, writing case notes, promoting services and developing  care solutions to enable our clients to maintain their independence in their own homes. You will be inspired by making a positive difference to our client base, coordinating quality services and providing exceptional customer service, you will be improving the quality of life for those we serve.

Qualifications

  • Reliable registered  motor vehicle
  • Comprehensive car insurance
  • Police Check (valid within the last 3 years), or willingness to obtain.
  • First Aid Certificate and CPR

Preferred Skills

  • Previous case management experience in aged care
  • Knowledge of the Community/Aged Care system and Govt funded care packages
  • Effective stakeholder management skills, engage effectively with all clients, families & carers and key stakeholders  
  • Excellent networking, consultation skills to develop new referral sources and relationships with appropriate external organisations.
  • Exceptional communication, customer service, negotiation and influencing skills
  • Strong attention to detail and good computing skills
  • Strong resilience and ability to work under pressure
  • A current valid driver's licence
  • Staff management/supervision preferred
  • APHRA Registration preferred
