Care Coordinator – Aged, Disability & Transitional Care

We are currently recruiting for a care coordinator, preferrably lives in the city or the eastern suburbs, to oversee our home care operation in the Sydney Eastern Suburbs area. Our services include, aged and disability care, transitional care, post-operative care and palliative care. Right at Home is a leading home care provider in the USA and is now launching its operations in Australia. This role is a full-time management position reporting to the Managing Director of Right at Home Sydney Eastern Suburbs operating from our office in Glebe area.

The role would involve coordinating and allocating your care team and ensuring that client needs are met. You will also be responsible for assessment and management of HCP and private clients.

• Driver’s License and comprehensively insured dependable private vehicle.
• Current National Police Clearance.
• AHPRA-Nurse Registration a requirement.

• Appropriate qualifications and demonstrated commitment to the Aged Care Sector and the Home and community care environment.
• The ability to think strategically.
• It is also essential to have good judgment, quick decision-making and excellent staff management skills.
• A good communicator.
• Proven ability to assess, plan, implement and evaluate care and services.
• Have an excellent understanding of education and accreditation standards, Continuous Improvement, OHS and risk management.
