We support people living with Alzheimer’s, dementia and cognitive decline, disabilities, post-operative care needs and older Australians. Qualifications

The Care Coordinator will be responsible for:

  • Building strong connections with clients and their families
  • Assigning staff to clients based on staff qualifications and availability, as well as client preferences
  • Organise support to clients with disabilities and monitoring and reporting on their progress
  • Referring clients to agencies that can provide additional help
  • Assessing client needs and resources for health, welfare and other facilities and services
  • Responding promptly to emails, enquiries and requests from clients, staff and management
  • Maintaining accurate records of incidents and feedback as required
  • Re-organising work priorities to meet the needs of a fast-paced environment
  • Supporting all clients to meet their goals
  • Conducting Client Care Assessments and Care Planning
  • Supervision and management of Care Staff
  • Discussing and promoting Right at Homes services with medical professions and referral sources
  • Reporting to head office
  • Ability to work in a fast-paced environment

Preferred Skills

At least 3 years’ experience as and Registered Nurse, Enrolled Nurse, Assistant in Nursing or 5 years + industry experience

  • Experienced in assessing and planning care needs
  • Sound understanding of the Aged Care and Disability systems
  • A strong client focus which is based on their goals, preferences and needs
  • Demonstrated experience managing staff
  • Current National Police Check
  • Working with Children Check
  • Full Drivers Licence
  • All candidates must have the right to work in Australia
  • Organisational, administrative and time management skills
